Space Request Form
Completion of the Space Request Form, with proper signatures and supporting documents, is required to start the space request process. It must have signatures from department FAO and department head before submitting to facilities management for review by the space committee and approval by the dean.
Exception Request for Office Space
This Exception Request for Office Space form must have signatures by the department chairs or senior associate dean or unit, then sent via email to Facilities Space Planning Department. They will be reviewed by the space committee before being signed off by the dean.
